Clinical Cohort
The Nebraska Stroke Association’s Clinical Cohort brings professionals together across those settings to strengthen communication, share resources, identify gaps, and improve connections throughout the stroke system of care.
Nurses, physicians, stroke coordinators, case managers, and others involved in acute stroke care.
Physical therapists, occupational therapists, speech-language pathologists, rehabilitation physicians, and therapy teams.
Outpatient rehabilitation, home health, primary care, behavioral health, and community-based providers.
Professionals working in communities where access to specialty stroke services may be limited.
Telehealth organizations, universities, researchers, support programs, and organizations working to improve stroke care and recovery.
The Nebraska Stroke Association Clinical Cohort is a statewide network of professionals and organizations involved in stroke care, rehabilitation, recovery, and community support.
The purpose of the cohort is simple: help Nebraska’s stroke professionals stay connected.
Stroke survivors often move through several different systems from emergency care and hospitalization to rehabilitation, outpatient therapy, primary care, and long-term community support. The Clinical Cohort creates a place for the people working in those settings to share information, identify gaps, and build stronger connections across the stroke continuum.

The cohort is not intended to replace hospital stroke committees, professional organizations, or existing healthcare partnerships. Instead, it provides a statewide connection point where those groups can learn from one another and work together when appropriate.
The Clinical Cohort is open to professionals and organizations whose work intersects with stroke care or recovery, including physicians, nurses, stroke coordinators, case managers, physical therapists, occupational therapists, speech-language pathologists, rehabilitation professionals, social workers, behavioral health providers, home health professionals, outpatient providers, critical access hospitals, telehealth organizations, universities, researchers, EMS agencies, and community partners.
You do not have to work exclusively in stroke care to participate.

The cohort is designed as a collaborative statewide resource. Participation does not require NSA membership or a financial commitment.

Our role is not to direct clinical care. Our role is to connect people, resources, and organizations across the stroke journey so that professionals know what is available and survivors and families have clearer paths to support.
